Enrollment Form

Please complete an Enrollment Form prior to arriving for your child’s first lesson. If your child is under the age of three or not potty trained, please bring a signed copy of our Swim Diaper Policy as well. Please note that in order to be successfully enrolled into one of our classes, you must also register for classes with our office staff and pay for the class that you choose.
